I'm not a Ghost Rider expert either, but I didn't think the Johnny Blaze Ghost Rider even had the Penance Stare; I was under the impression that only the Danny Ketch Ghost Rider had that power.

So am I. The few books on Ghost Rider I had never showed Blaze performing that trick, while Ketch did it four or five times. I don't know if he (Blaze)wasn't able to do it, but he didn't in those books.
Personally, I would put his fighting skills at 6 (close)/ 5 (ranged) in Ghost Rider form, and add an option that allows him to use his Vehicle Operations as Acrobatics when he's on his bike (kinda like the Aerial Acrobatics of Vulture in the Spidey Guide).

Originally Johnny Blaze did not have the Penance Stare. In fact over all his powers were a little different from his predecessor. Danny Ketch has the Penance Stare and used it frequently. In the beginning it was like a wrestler's finishing move. Recently, with the return of the Johnny Blaze/Ghost Rider combination, the Ghost Rider has retained this ability and has used it.

You can find this and a few other things in the "Unofficial Spider-Man Book Discussion". I've done a little research on Ghostrider in the past b/c I made a character (my first one based in this system) surrounding the character. In the comics, Blaze does not produce the Penance Stare. As far as Madcap and insanity goes, I'll try to remember to take a look at the comic and I'll post back if there any answers there.

Blaze now has the Penance Stare and I just read the issue when Ghost Rider gave the Stare to Madcap. It twisted Madcap even more. Before that, Madcap could feel no pain. Afterwards, Madcap not only could feel pain, but got kind of turned on by it. It ends with him wanting more.

That's post Ghostrider Blaze though, right? I know that in the comics he seems to be able to produce some of the same effects as he could when he was Ghostrider. Like, I think he could use guns imbued with hellfire, for instance, and produce chain possibly. Back when I created the character (near the beginning of this year) I used wiki as a source so that would be a good place for anyone looking to create the character probably. I'd actually be really interested in seeing a post-Zarathos Blaze (I always found that character interesting).

In the latest series Blaze is Ghost Rider and he has the stare. He seems more like the Ketch Rider. The old GR was of a different flavor all together.

Does ghost rider have any mental def? Should the rating of the penance stare (telepathy) be used as mental def modifier?
I've just been wondering about that whole telepathy affecting demons like Ghost rider and Blackheart.
Anyone?

I always thought that Telepathy can't exactly work on alien minds. Demons could be considered alien, but characters like Ghost Rider have a human anchor. That human part would be telepathically accessible, with no Bonuses to protect it.
