I really like this idea but I would like to point some things that look out of place.
Transformations through masteries like Iceman's and Human Torch shouldn't be counted as a Transform Self challenge.
Both of them could use their force blasts and manipulate their elements and both had to be transformed to gain their flight capabilities (although not sure about Iceman).
IMO they should just get an extra -1 discount for that disadvantage at their flight action.
Now since having two powerful forms is the main problem I suggest the following.
You wish for a hero/villain with two or more forms, you have to pay for the highest ability/action/modifier between forms.
F.E. Hulk/Bruce Banner has 7 intelligence as Banner and 1 strength but as Hulk he has 1 intelligence and 10 strength, paying for both forms and then taking a discount for the weakest sounds lame to me.
Why not go directly for 7 intelligence and 10 strength from the beginning and don't bother at all for transforming and stuff.
What I'm trying to say is that if you have similar actions between forms you have to pay only once for the highest action number between them it won't hurt to have the same action in 5 different transformations as long as they are equal or lower to the one you bought it, therefor you don't need a discount at all.
If you have an action that you cannot have in any other form like Bruce Banner's Invention (Hulk isn't smart enough to use even a doorknob, how much invent something.) then you deserve a discount because you're limiting your character a lot (even one panel of actions counts and from my experience transforming counts as an action).
Also you can give an extra discount by using the limit of actions on your CAD at your advantage. One could simply ask for 2 forms with 6 actions on the first and 5 more on the other with only one being actually the same (therefor sharing a box in the CAD) effectively he's cheating the limit even though it might sound logical to have more actions than nine.
This is where you should make the Transform Self an advantage give him the extra action box but cost double in stones for that particular box and it won't be shared between form nor get a discount for that particular action.
I won't suggest how many stones should be rewarded, I'll leave TWF do that since he's more experienced than me in such matters.
